New supply calculator and request forms make refills easy at Village

Do you constantly wonder when you might need to refill your prescription? And, when you need your fertility meds, would you prefer an alternative method to picking up the phone and requesting a refill?

At Village Fertility Pharmacy, our goal is to make this experience as easy as possible for you and we’ve recently implemented two new tools on our Web site that should make calculating your remaining supply of medication and requesting a refill a whole lot simpler.

Requesting a refill:

Check out our new, downloadable refill form that can be quickly filled out and submitted via the Web. We’ve added this form to our Web site to offer convenience to those who prefer using the Internet for these kinds of transactions. Of course, if you still prefer making a phone call and speaking to one of our representatives, you can still do that, too.

You will need to have Adobe Reader installed on your computer in order to open and read the form. If you do not have this software, you can download it for free here.

In order to complete your refill request, you must also have the prescription number, which can be located on your medication label. If you cannot find your prescription number or have trouble using this form, please do not hesitate to contact our Patient Care Center at (877) 334-1610.

Calculating your remaining supply of fertility medications:

As you know, assessing how many days worth of medication you have left can sometimes be difficult, but it’s imperative that you anticipate your needs as best you can.

By utilizing our brand new Medication Supply Calculator, you can gauge when your next refill may be needed to help you better prepare for the days ahead and avoid stressful emergency situations. In order to evaluate your medication supply, you must enter the following information into the form:

• Drug name
• Medication package size in International Units (IUs)
• Number of packages you have on hand
• Number of IUs you use for each dose
• Number of times you take the medication each day
• Number of days you need to take the medication (including today)
